1) Hyde Parking-lot Road
2) Wharncliffe Road
3) Boler Road/Sanatorium Road
4) Highbury Avenue
5) Oxfart Street
7) Western Road (South of Sarnia)

lol a bit biased towards Toronto don't you think? 9 of the top 10 are there.

There are surely worse roads elsewhere in the province, but more people vote for their local streets.

Voting population plays a role, but IMO Toronto roads are substantially shittier than just about anything else in the province. Driving along Wharncliffe is like being on a ribbon of silk compared to most Scarberian arterials.
