Retail is most definitely required on all major avenues in Midtown.
There are a couple exceptions uptown (Park north of 60th, West End North of 72nd, parts of Riverside, CPW and Fifth), but ground floor retail is required pretty much every else.

Fifth Avenue north of Midtown is one of the exceptions. This building is being built in the middle of Midtown, where there are no exceptions.
There are essentially no exceptions south of 59th Street, except for a short stretch of Fifth Ave. north of Washington Square.
