I know that in Vancouver, before the train cars can go into service, they must run as "ghost trains" (trains with no passengers and are driverless and runs and night [still stop at stations]) for 2,000 kilometres as part of their extensive testing before going into active service.

I think it is where the problem come from. They did not have enough testing.
However, it may be that company's fault....
Not sure about this incident
